  1. Eligibility: This offer is available only to new Home Care clients of  Vitalis Health and Home Care Pty Ltd (“Vitalis”, “Vitalis Aged Care at Home”), ABN 96 607 394 972, who are either:

    – Private (self-funded) clients – those paying for their home care services out-of-pocket (no current government subsidy); or|

    – Support at Home (SaH)- funded clients.

     

  2. Offer Details – Credit Amount: Eligible clients will receive a one-time credit applied towards their Vitalis Home Care service fees, or SaH co-contributions, valued up to $1500.

    The exact credit amount will be determined after an assessment and will be based on the number of hours of Home Care per week in your plan, provided the plan covers at least two (2) hours per week.

    Every eligible new client will receive a minimum $250 credit. Clients with a higher number of weekly care will receive a larger credit, up to $1500.

    This promotional offer is not available to clients whose care is assessed by Vitalis as being of a short-term nature. The exact credit amount will be confirmed in writing after the initial assessment.

     

  3. Active Service Requirement: To receive the credit, you must remain an active Vitalis client for at least three (3) consecutive months from the start of services. The average weekly actual hours of home care services at the end of the period must match or exceed the weekly hours set in the initial care plan in your contract.


    The credit will be forfeited if services are discontinued or your Home Care Agreement is terminated before the end of the third month.

     

  4. Application of Credit: The credit will be applied as a one-time deduction on your third month’s invoice.

    For privately funded clients, the credit will simply reduce the amount you owe on that invoice.

    For SaH-funded clients, the credit will be applied against your out-of-pocket co-contributions due in the third month. If the credit amount exceeds what you owe in that month, any unused credit will roll over to subsequent invoices until fully used, provided you remain an active client. The credit cannot be applied to fees covered by government funding or to any third-party charges. It only offsets the portion of costs that you, the client, would normally pay.

     

  5. Funding and Compliance: This promotional credit is funded entirely by Vitalis as a goodwill offer to clients. It does not affect your Support at Home funding entitlements or reduce your package funds in any way. All services under this offer are delivered in accordance with the Aged Care Quality Standards and program guidelines. The offer is structured to ensure compliance with Australian Consumer Law and Aged Care regulations:


    Transparency: The actual credit will be calculated following initial assessment, development and approval of care plan. You will not be asked to increase services just to get a higher credit, in line with our obligation to deliver care appropriate to your needs (Aged Care Code of Conduct, Integrity and Honesty).

    – No Impact on Care Quality or Package: You will receive the same high-quality care and full use of your Home Care Package funds. The credit simply means Vitalis covers part of your cost for a period. Government subsidies to your package remain unaffected.

    No Government Endorsement: This offer is a private promotion by Vitalis Health and is not sponsored or endorsed by any government agency. It is an added benefit we provide and does not constitute an official entitlement of the Support at Home program.


  6. Service Inclusions and Exclusions: The credit applies toward Vitalis Home Care services falling into Independence Services or Everyday Living categories included in your care plan. It cannot be applied to:


    – Services or products from third parties or outside providers (e.g. equipment purchases, medications, etc., if not provided by Vitalis).

    – Clinical Services

    – Any government fees or charges (if applicable).

    The credit  cannot be redeemed for cash, and any unused portion of the credit has no cash value. The credit is not transferable to other individuals or programs.


  7. How to Claim: The credit amount you qualify for will be confirmed after your initial home visit assessment. You must then sign up for services (execute the Home Care Agreement) within seven (7) calendar days of that assessment to lock in the offer. Once you meet the eligibility and active service requirements, the credit will be automatically applied to your invoice for the third month of services, which will be received in month 4.

    You do not need to submit any additional claims or paperwork. The credit will be clearly shown on your bill.


  8. Offer Duration: This is a limited-time offer valid for new client assessments completed and agreements signed by 30 June, 2026.

    Vitalis reserves the right to end or modify the offer earlier at its discretion, for example, if regulatory conditions change. Any such changes will be updated in these Terms and Conditions on our website. Clients who have already signed up and qualified prior to a modification or withdrawal will still receive any credit that has been confirmed in their agreement.

  9. General Conditions:

    – This offer cannot be combined with any other promotional offer or referral bonus from Vitalis. If you are eligible for multiple promotions, the one with the highest benefit will be applied.

    – The credit will be documented in an attached schedule to your Service Agreement, including the amount and how it will be applied, in line with aged care pricing transparency requirements.

    – Participation in this offer does not waive any rights you have under the Aged Care Act or Australian Consumer Law. This promotion is intended to enhance your benefits as a client in a compliant manner.

    Dispute Resolution: Any disputes or complaints regarding this offer will be handled in accordance with our standard complaint management process and relevant regulatory requirements. You may also contact the Aged Care Quality and Safety Commission or ACCC if you believe your rights have been affected.

    Vitalis reserves the right to update these Terms & Conditions as necessary to ensure legal compliance or enhance clarity. The most current version will be posted on our website, and existing clients who have enrolled under this offer will be informed of any significant changes.
